Higher Level Advanced Clinical Practitioner - Acute Medicine

NHS AfC: Band 8b

Job overview

The clinical 8b Advanced Clinical Practitioner (ACP) in Acute Medicine is a senior autonomous clinician who provides advanced assessment, diagnosis, treatment, and management of patients presenting with acute, complex, and undifferentiated medical conditions across the acute care pathway. The post holder will demonstrate expert practice within the four pillars of Advanced Clinical Practice: clinical practice, leadership and management, education, and research.

Acute Medicine at SaTH has undergone significant transformation over the past five years, resulting in the growth and development of a well-established Advanced Clinical Practitioner workforce. We are seeking a highly motivated and experienced individual to join the team in this senior clinical 8b ACP role to support the on going work across the Acute Medicine areas and whilst working towards delivery of our Hospital Transformation Programme in 2028.

Working at an advanced level of practice, the post holder will provide expert clinical care across Acute Medical Wards, Same Day Emergency Care and ACP-led outpatient services. They will independently undertake comprehensive patient assessments, formulate differential diagnoses, initiate and interpret investigations, prescribe and review treatments, develop management plans, and coordinate multidisciplinary care to optimise patient outcomes and patient flow.

Main duties of the job

While the overall clinical responsibility for patient care lies with the Consultant named for a particular patient, the higher-level ACP will act on behalf of the Consultant assessing, accessing diagnostics and interpreting findings formulating treatment plans for patients.

The clinical 8b ACP will provide strategic and professional leadership for Advanced Clinical Practice within Acute Medicine, supporting the continued development of innovative services and models of care.

A key element of the role will be the leadership and development of ACP-led services, ensuring delivery of high-quality, evidence-based care while supporting service sustainability and organisational objectives. The post holder will provide supervision, mentorship, and professional support to trainee and qualified ACPs and contribute to the education and development of the wider multidisciplinary workforce. The higher level ACP will receive clinical supervision from our experienced Acute Consultant Team whilst being supported to work on their own professional development. They will work closely alongside the Acute Medicine MDT which includes Medical, Nursing, Operational and administrative staff.

Working collaboratively with colleagues across acute services, the clinical 8b ACP will play a pivotal role in advancing clinical standards, improving patient experience, supporting operational performance, and shaping the future development of Advanced Clinical Practice within the organisation.
