This role is being advertised on behalf of Littleborough Group Practice and the successful applicant will be directly employed by Littleborough Group Practice. We are seeking a dedicated and organised Care Navigator to join our lovely reception team at Littleborough Group Practice. The successful candidate will play a vital role in coordinating patient care, providing administrative support, and ensuring seamless communication between patients and healthcare providers. This paid position offers an excellent opportunity for individuals with strong organisational skills and a passion for delivering compassionate care. The ideal applicant will possess relevant office experience and be proficient in various computer applications. A higher hourly rate provided depending on experience.

Main duties of the job

  • Act as the primary point of contact for patients, providing courteous and professional assistance via phone and in person.
  • Manage patient appointments, scheduling, and follow-up communications efficiently.
  • Maintain accurate records through data entry and administrative tasks using clinical systems EMIS and DOCMAN.
  • Assist with processing paperwork, billing, and insurance documentation.
  • Coordinate with healthcare providers to facilitate patient care plans.
  • Ensure all communication adheres to confidentiality standards and organisational policies.
  • Utilise organisational skills to prioritise tasks effectively in a fast-paced environment.
  • Perform general clerical duties including typing, filing, and maintaining office supplies.

Littleborough Group Practice is a CQC-rated Good, EMIS Web practice caring for approximately 8,600 patients. We are a training practice and a member of Pennines PCN.

About Littleborough

Littleborough is a picturesque market town on the edge of the South Pennines, with Hollingworth Lake and miles of moorland on the doorstep yet only 30 minutes by train to Manchester and a short drive from the M62.

Job responsibilities

  • Act as the first point of contact for patients, providing courteous and professional assistance in person, by telephone and via online channels.
  • Use active signposting and care navigation to direct patients to the most appropriate service, clinician or external support.
  • Book, amend and cancel patient appointments accurately, managing the appointment system efficiently.
  • Handle patient queries and requests sensitively, escalating to clinical or senior staff where appropriate.
  • Maintain accurate and up-to-date patient records using the clinical systems EMIS Web and Docman.
  • Process incoming and outgoing correspondence, registrations, and information requests in line with practice procedures.
  • Carry out general clerical tasks including data entry, scanning, filing and document management.
  • Support the smooth running of the reception area, including monitoring and ordering office supplies as needed.

Communication and teamworking

  • Communicate clearly and effectively with patients, colleagues and external healthcare providers.
  • Work collaboratively as part of the wider practice team to support coordinated patient care.
  • Prioritise and manage a varied workload effectively within a busy, fast-paced environment.

Governance and confidentiality

  • Ensure all communication and record-keeping adheres to confidentiality standards, data protection legislation and practice policies.
  • Comply with all practice policies and procedures, including those relating to information governance, infection prevention and control, health and safety, and safeguarding.

Person Specification

  • Proven office or administrative experience with strong organisational skills.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • Excellent telephone manner and the ability to communicate clearly and professionally.
  • Strong data entry skills with a high level of accuracy and attention to detail.
  • Ability to manage multiple tasks and prioritise effectively under pressure.
  • A warm, compassionate and patient-focused approach.

Experience

  • Experience using EMIS Web (1 year).
  • Experience using Docman.
  • Previous experience in an administrative role within primary care or a related sector.
  • Previous clerical experience (highly motivated candidates willing to learn are encouraged to apply).
