About the role: We are seeking an Advanced Practitioner to be appointed to the Preparing for Adulthood (PFA) Learning Disabilities and Autism Team. The team support adults, families and informal carers live as an enjoyable and independent life as possible by working in a strength‑based way to listen to and empower individuals, identify their outcomes and explore ways of using their own strengths. This is an exciting opportunity for a successful candidate as you will work alongside the Team Manager in developing the new team and co‑producing with service users, families, carers and people with lived experience to ensure service delivery is outcome focused and incorporates best practice. As part of the wider Adult Social Care service you will also work closely with the five integrated neighbourhood teams, front door team, and Adult Mental Health Services supporting and learning from each other. This is an exciting time in Rochdale as the service implements service redesign through our transformation agenda and Corporate Plan.
About you: You will have:
- Honours Degree or Diploma in Social Work
- Registration with Social Work England.
- Line management experience
- Someone who is positive, proactive and creative to make a difference in the lives of people with learning disabilities and autism.
