We are looking for amazing people who share our vision and values to join our team and have some fantastic opportunities for a motivated and forward-thinking Social Worker to join one of our teams. We currently have Social Work opportunities for experienced Social Workers in the Integrated Neighbourhood Teams. The salary on offer is £40,777 - £47,181 (Grade 8).
Our Front Door and Prevention Team supports people at the earliest opportunity to maximise the effectiveness of early intervention and prevention. The Adult Front Door Service is the first point of contact for individuals seeking health, social care and wellbeing support and is responsible for Adult Care’s initial care, prevention and risk management response. The team consists of a range of practitioners including social workers, support planners, prevention workers and triage assistants with a focus on multi-agency collaboration to provide a person-centred response to all contacts to effectively support individuals and carers to live well, safely and as independently as possible.
Our integrated neighbourhood teams are at the heart of our adult care service, delivering and reviewing innovative services for our residents, helping to reduce health inequalities, enhance life and support independent living. We work across friendly, dynamic teams alongside our district nurse colleagues with a focus on partnership working to achieve best practice and desired outcomes for our residents. We take pride in everything we do to ensure our services continue to be of an excellent and high-quality standard.
Our Hospital Discharge Flow Hub supports Rochdale residents with discharge arrangements, timely and safely to a suitable setting. We utilise various Discharge to Assess and re-ablement pathways to support individuals to maximise their independence away from the hospital setting, with a focus on discharge to the right place at the right time. The team is responsible for working with people to assess both the immediate and ongoing needs, in order to support a person in implementing their longer-term care and support needs. The team is located within a central community-based office but will support discharge planning across the various hospital sites in and out of the borough as part of a multi-agency team to facilitate effective, safe and timely discharges.

The postholder will be expected to undertake a certain amount of travelling in the course of his/her duties for which a casual car user allowance will be paid. Hours may be required to be worked over 7 days, as per the needs of the service.
In accordance with Section 7 of the Immigration Act 2016 this post requires the ability to converse at ease with the members of the public and provide advice in accurate spoken English. The Council is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects staff to share this commitment.
Casual User Car Allowance is payable. Appointment to this post is subject to an enhanced DBS including a barred list check adult workforce. Interviews to be held on Wednesday 16th September within the Middleton Locality based at Brook House.
