About the Role

We are seeking a dedicated and compassionate Outreach Support Worker to join a supported services team in Wakefield. This is a rewarding opportunity to make a real difference in the lives of vulnerable individuals by supporting them on their journey towards independence and long-term stability. Working on a rolling rota, including early starts and weekends, you will provide practical and emotional support to customers, helping them overcome barriers and access the services they need to rebuild their lives.

Key Responsibilities

  • Support customers in accessing employability services, training opportunities, and specialist support agencies
  • Assist with budgeting, money management, and financial planning
  • Provide guidance with benefit applications and welfare support
  • Help customers secure and sustain suitable long-term housing solutions
  • Maintain accurate records, case notes, and support plans
  • Build positive, professional relationships with customers while maintaining appropriate boundaries
  • Work collaboratively with internal teams and external partner organisations

About You

To be successful in this role, you will be resilient, empathetic, and committed to supporting individuals facing challenging circumstances. You will have excellent communication skills and the ability to engage effectively with a diverse range of customers.

Essential Requirements

  • Full UK driving licence
  • Access to a vehicle
  • Ability to work flexibly across a rolling rota, including weekends and early morning starts (from 5:30am)
  • Strong organisational and record-keeping skills
  • Experience supporting vulnerable individuals is desirable but not essential
