DaVita Integrated Kidney Care (IKC) – Southern Atlanta Metro / Poplar Dialysis is seeking a Care Coordinator to join our patient village and coordinate care for complex patients.
This full-time role is Monday–Friday with 3–4 clinic days onsite and remaining time remote; start between 7:00–8:00 AM; mileage reimbursed; home office must be quiet with high-speed internet; IT setup provided.
