We are seeking a compassionate, proactive, and motivated individual to join our team as a Social Prescriber / Health & Wellbeing Coach. This dual role offers an exciting opportunity to support patients in improving their physical, mental, and social wellbeing through personalised coaching, behaviour change support, and connection to community-based services and resources.

Key Responsibilities

  • Conduct person-centred conversations to identify individual needs, strengths, and aspirations.
  • Support patients experiencing social, emotional, practical, or non-medical challenges affecting their health and wellbeing.
  • Develop personalised support plans and connect individuals to relevant community groups, activities, and services.
  • Build and maintain strong relationships with local voluntary, community, and statutory organisations.
  • Advocate for patients where appropriate and support them to access services independently.
  • Maintain accurate records and demonstrate outcomes using agreed reporting systems.
  • Deliver personalised health coaching interventions to support behaviour change and self-management.
  • Support patients with long-term conditions to improve confidence, knowledge, and self-care.
  • Use evidence-based coaching techniques including motivational interviewing and goal setting.
  • Work collaboratively with patients to identify barriers to change and develop achievable action plans.
  • Promote healthy lifestyles and support improvements in areas such as physical activity, nutrition, smoking cessation, stress management, and overall wellbeing.
  • Contribute to reducing health inequalities and improving patient outcomes.

Qualifications

  • GCSE grade A to C in English and Maths.
  • Qualified to NVQ level 2 in Health and Social Care.

Experience

  • Experience working within healthcare, community, wellbeing, social care, or voluntary sector settings.
  • Excellent communication and interpersonal skills.
  • Ability to build trusting relationships with individuals from diverse backgrounds.
  • Understanding of person-centred care and behaviour change approaches.
  • Experience supporting vulnerable individuals and managing complex needs.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organisational, IT, and record-keeping skills.
  • Social Prescribing Link Worker training or relevant experience.
  • Healthcare Assistant (HCA) qualification and/or experience in a clinical setting.
  • Knowledge of local community resources and support services.
  • Experience working within Primary Care, NHS, or Primary Care Networks.
  • Training in motivational interviewing, health coaching, or behaviour change methodologies.
