JOB DESCRIPTION

  • River Exe Healthcare Care Service Facilitator
  • Reports To: Registered Manager

Location: River Exe Healthcare, local office dependent on vacancy. However you may be required to support other office locations in line with business needs.

Main Purpose of the Role

To provide high quality care coordination and administrate duties to support the day to day operations of the office and service.

The Care Service Facilitator will be responsible for staff rostering, shift allocation, managing staffing levels, maintaining accurate records, supporting compliance processes, and ensuring continuity of care for service users.

The role will work closely with the Registered Manager, care staff, service users, and external professionals to maintain high standards of service delivery and operational performance

  • Care Coordination & Roster Management
  • Create and manage staff rotas.
  • Allocate shifts and ensure care packages/ care runs are fully staffed.
  • Manage sickness, annual leave and emergency cover.
  • Monitor staffing levels and identify rota gaps.
  • Send schedules and rosters to the service users.
  • Administrative & Financial Management
  • Maintain accurate records and management systems.
  • Support payroll through accurate timesheet and rota information.
  • Assist with reporting and compliance documentation.
  • Support in the management of additional financial functions such as debtors, and payroll.
  • Assist with compliance checks.
  • Maintain training and certification records
  • Collate required information in line with weekly and monthly deadlines.
  • Participate in quality assurance and continuous improvement initiatives.
  • Service Standards
  • Respond to telephone and email enquiries.
  • Use care management and rostering software.
  • Maintain confidentiality and service user rights.
  • Engage with staff and service users over the telephone, handling queries, and directing them to the relevant team member.
  • Helping to resolve day to day queries from staff or service users.
  • Develop good working relationships with key stakeholders and other healthcare professionals.
  • Follow up with District Nurses (DNs) and General Practitioners (GPs) as required
  • Liaise with care staff, clients, external professionals, and third-party organisations

Qualifications & Skills

  • Experience in care coordination, scheduling or healthcare administration.
  • Strong organisational and communication skills.
  • NVQ Level 3 in Health & Social Care desirable.
  • Experience in financial record-keeping and office management.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Working Hours Full-time, with flexibility as required by the business.
