An exciting new opportunity has arisen for a motivated Nurse to join our dynamic, forward thinking Aintree diabetes nurse team. This post holder will work specifically within the Surgical Directorate supported by the wider Diabetes nursing team. As a Diabetes Surgical Nurse Educator your role will be to support high quality safe care to patients with diabetes. You will be working with the wider diabetes multidisciplinary team to provide specialist diabetes support, advice and education for patients with type 1 and type 2 diabetes as well as healthcare professionals.
We are seeking a nurse who has a positive approach and is committed to making a difference. You will need to demonstrate a keenness to work in the field of diabetes, effective communication and teamwork. Experience of supporting patients and carers with diabetes management would be an advantage but not essential as training and development in role will be provided. Applicants must have good communication skills, and have a flexible approach to working hours. Applicants should be able to demonstrate criteria as set out within the job description and person specification. Successful applicants will receive training to support them in the role.
This band 6 Surgical Diabetes Nurse Educator role will include the following:
- The post holder will provide on-going specialist education and training for patients, carers and staff around safe management of diabetes care.
- The post holder will be the main link for diabetes nursing within the Surgical Directorate and will visit these ward and clinical areas daily (Monday to Friday) develop a collaborative working relationship and offer daily support to the surgical nurses, doctors and AHPs.
- To provide specialist nursing advice, in a format appropriate to the individuals needs/abilities, to professionals, people with diabetes and carers in all aspects of diabetes care.
- Provide advice using evidence based/patient centred principles to assess, plan, implement and evaluate routine and specialist interventions with the support of a management plan.
- To be accountable for safe and appropriate advice around medications used commonly within the speciality of diabetes care.
- To support the diabetes team in planning, development and evaluation of diabetes services across the locality.
- To be a resource for specialist advice and information across the organisation.
Working closely with the Diabetes Specialist Nurses to perform comprehensive assessment of patient nursing needs, plan, implement and evaluate care delivery according to changing health care needs. Providing individualised education to patients and carers to support diabetes management and promoting healthy lifestyle. Collect, collate, evaluate and report information, maintaining accurate patient records. Involve patients and carers/relatives in the planning and delivery of care. Clinical sessions in a variety of settings mostly ward based reviews. Supporting safe effective discharge from hospital.
Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to ongoing care needs and discharge arrangements. Establish and maintain effective communication with patients and carers/relatives.
Managerial/Leadership:
- Provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives.
- Undertake clinical supervision of nursing colleagues on an individual or group basis.
- Participate in the development of services and assist the Nurse Consultant to implement change.
- Ensure the effective and efficient use of physical and financial resources.
- Monitor health, safety and security of self and others and promote best practice in the ward area.
Education and Development:
- Provide education and development opportunities for others including patient education and education of other health care professionals.
- Develop own expert clinical knowledge and participate in the development of others clinical knowledge.
- Ensure own compliance and compliance of others with regard to mandatory training and revalidation requirements.
Clinical Governance:
- Registered Practitioners have a responsibility to maintain current active professional registration.
- Act always in accordance with their professional Code of Conduct and guiding documents.
- Adhere to Trust Policy and Procedure.
- Maintain up to date skills and knowledge and maintain awareness of professional issues.
- Maintain a professional portfolio.
