The Health Services Director works with the Wellness Director and Wellness Staff, including Personal Care Attendants, Certified Medication Aides, Certified Nursing Assistants, Registered Nurses, and Licensed Practical Nurses. This role ensures that resident care plans, regulations, and corporate policies are adhered to, while maintaining the safety and comfort of residents. The Director oversees medication control, pharmaceutical distribution, and medication monitoring for all residents.
Responsibilities:
- Responsible for resident’s clinical oversight and coordination of care.
- Plan, develop, organize, implement, evaluate, and oversee the Medication Administration Program ensuring compliance with current rules, regulations, and guidelines.
- Maintain an active presence on the floor with staff.
- Foster positive relationships with residents and their families.
- Ensure timely follow‑up on clinical issues or concerns.
- Participate in setting goals and objectives for medical and pharmaceutical systems; recommend and implement necessary changes.
- Manage Medication Administration Records (MARs) and oversee the delegation of medication assistance and administration.
- Review pharmacy reports, dietician reports, and physician’s orders according to standard operating procedures.
- Coordinate third‑party ancillary services and manage transfer agreements, when necessary.
- Ensure seamless transitions for admissions and re‑admissions by coordinating with hospitals and SARs; recommend and implement changes when necessary.
- Assess new residents’ conditions according to policy and discuss care coordination with the management team.
- Demonstrate a thorough understanding of resident rights and uphold them consistently.
- Maintain a strong knowledge of HIPAA regulations and ensure compliance for yourself and your staff at all times.
- Assist in developing and implementing plans of action to address and correct identified or potential deficiencies.
- Conduct in‑services, and coach and develop the team in a positive, energetic, and compassionate manner to ensure resident needs are met and all Human Resources and facility policies are followed.
- Monitor the timely completion of all documentation, including incident reports and health care plans, and coordinate all incident investigations and related documentation.
- Ensure continuity of residents’ care, assist in evaluating their need for physician referrals, community services, or alternate levels of care.
- Provide the Executive Director with updates on residents’ health assessments and the department’s capacity to meet their needs.
- Conduct monthly clinical in‑services for staff.
- Foster a positive work environment by serving as a supportive leader to the team and warmly welcoming new hires.
- Conduct employee orientation and in‑service education and training for the staff and residents as required by the facility.
- Assist in developing, revising, and updating infection control policies, in‑service training courses, etc.
- Provide quality medical care to residents, prepare documentation for annual surveys, and report and investigate any claims of abuse, neglect, infectious disease, or other concerns as needed.
- Perform administrative duties, including completing medical forms, reports, evaluations, studies, and charting as necessary.
- Assist with the evacuation of individuals and provide support in the event of falls or other emergencies.
- Develop initial and comprehensive assessments of residents’ daily living needs in a timely manner, as required by the facility.
- Organize, develop, and conduct service plan reviews in accordance with state codes, involving appropriate resident care team members and resident families.
- Supervise the maintenance of resident charts, review documentation prepared by care staff, and ensure accuracy and timeliness of entries.
- Participate in surveys and inspections conducted by authorized governmental agencies or consultants, as required by the Executive Director.
- Serve on, participate in, organize, and/or attend various functions for staff and residents as directed by the Executive Director.
- Coordinate a monthly wellness activity for the residents.
- Participate in recruitment of wellness staff, conduct evaluations, and handle disciplinary actions.
- Collaborate with the Executive Director to carry out corrective actions as needed.
- Share on‑call responsibilities as required.
- Attend in‑service programs provided by the company and external agencies related to position and regulatory information and updates; in addition to fulfilling education requirements for keeping nursing licensure in good standing.
- Perform other related duties as assigned.
Qualifications:
- Possess a valid RN license in the state of employment.
- Have at least 2 years of supervisory experience, preferably in an assisted living environment.
- Demonstrate a thorough understanding of nursing and medical practices, including laws, regulations and guidelines that pertain to residential and assisted living communities.
- Exhibit excellent written and oral communication skills, with the ability to write clearly and understandably in English to ensure staff and medical practitioners follow instructions accurately.
- Show the ability to deal tactfully with residents, their family, regulatory agencies, personnel, and the general public.
- Display patience, tact, a cheerful demeanor, enthusiasm, and a willingness to handle challenging situations residents.
- Have strong skills in planning, organizing, developing, implementing, and interpreting program goals, objectives, policies, and procedures.
- Be proficient in working with computerized systems, such as EMAR.
