Location: Right at Home Sefton, Unit 39A Space Solutions Business Centre, Sefton Lane Industrial Estate, Maghull, Liverpool, L31 8BX

Working Hours: Full-time - 32.5 hours per week, 9:00 AM – 4:00 PM, Monday to Friday (on-call shifts will be additional as part of the role)

Rate of Pay: Β£14.08 per hour

Role Overview: As a Care Coordinator, you will be responsible for coordinating and managing care services for our clients. This is a key role in ensuring that our clients receive the highest standard of care tailored to their specific needs. You will oversee scheduling, support the team of caregivers, and act as the first point of contact for both clients and staff. The role will also involve providing on-call support outside of regular working hours.

Key Responsibilities:

  • Care Planning & Coordination: Develop and maintain personalized care plans for clients, ensuring they are tailored to meet their individual needs, preferences, and goals.
  • Scheduling & Staff Management: Schedule caregivers to ensure clients receive timely and appropriate care, adjusting for any changes as needed. Manage and maintain accurate records for all shifts and staff members.
  • Client & Family Communication: Maintain regular communication with clients and their families to ensure satisfaction with the care being provided. Act as a point of contact for any concerns, queries, or feedback.
  • On-Call Support: Provide on-call support during evenings, weekends, or outside normal working hours on a rotating basis, ensuring that all client care is met in a timely manner.
  • Caregiver Support & Supervision: Offer guidance, support, and supervision to caregivers, ensuring they adhere to care plans and follow best practices. Conduct regular performance reviews and offer constructive feedback.
  • Compliance & Quality Assurance: Ensure that all care is delivered in compliance with regulatory standards, company policies, and procedures. Monitor and report on the quality of care provided to clients.
  • Documentation: Maintain accurate records of care plans, schedules, incidents, and client needs. Ensure timely updates to client records in line with any changes.

Skills & Qualifications:

  • Previous experience in care coordination or a similar role within the care industry is preferred.
  • Strong communication skills, with the ability to liaise effectively with clients, families, and staff.
  • Excellent organizational and time management skills.
  • Compassionate and person-centred approach to care.
  • Ability to manage multiple priorities and problem-solve under pressure.
  • A good understanding of regulatory requirements and care standards.
  • Full UK driving license and access to a vehicle is essential.
  • Willingness to be flexible and provide on-call support as part of a team rotation.
