We are currently recruiting for a Band 7 Clinical Pharmacist – Frailty and Community Hospitals to work on an ongoing basis in South West London. Please see details of the role below:
- Role: Band 7 Clinical Pharmacist – Frailty and Community Hospitals
- Days: Monday to Friday
- Hours: 9am – 5pm
- Start: ASAP
- Length: 3 months
As a key member of the multidisciplinary team (MDT), you will provide clinical pharmacy input across community hospital services. Responsibilities include reviewing and optimising medicines for patients living with frailty and polypharmacy, supporting service development by contributing to guidelines, policies, and procedures, and ensuring safe and effective medicines use across community services. You will also benefit from rotation within the wider Trust, gaining exposure to a range of specialties and further developing your clinical, leadership, and managerial skills. Please note you will be required to work across different sites each week.
Requirements:
- Minimum of 6 months NHS experience
- Right to Work in the UK
- GPhC registration
- Clinical knowledge and experience ideally within frailty, older people, or community hospital services
- Strong communication and interpersonal skills with the ability to work effectively in an MDT
- A proactive and flexible approach with a commitment to evidence-based, patient-centred care
