We are currently recruiting for a Band 6 Community Nurse for our client based in the Stoke-on-Trent area on a 3 month contract. This role will involve working on the Home First team over the days of Monday-Sunday, 9am-8pm.

Duties involve:

  • To take a lead role in the clinical assessment of people in their own home environment
  • Provide advice, support and guidance to colleagues and other professionals on issues relating to Home First.
  • To co-ordinate patients’ care
  • Advise manager of any issues that may affect the Home First team
  • Take on Home First referrals in line with referral criteria
  • Participate in the planned induction, training and support of other members of the team

Rates:

  • Monday-Friday Days: £26ph
  • Saturday/Nights: £34ph
  • Sundays/Bank Holidays: £41ph

Essential Requirements:

  • NMC Registered
  • Previous experience within a nursing role
  • Car Driver
