Reporting To: The Partners and Practice Manager
Job Summary: Deliver high‑quality primary care services as part of the Primary Care Network (PCN). The role supports improved patient access, health outcomes and system efficiency in line with the Network Contract DES, with a focus on capacity and access.
Main Responsibilities:
- Provide a full range of GP services, including face‑to‑face consultations, telephone and video consultations, and home visits where appropriate.
- Diagnose and manage acute and chronic conditions.
- Prescribe in accordance with local and national guidelines.
- Maintain accurate and contemporaneous clinical records.
- Support the practice using online consultation tools and Total Triage.
- Carry out on‑call duties as required.
- Support enhanced access and same‑day care provision.
- Contribute to holistic care, QOF achievement and structured medication reviews where appropriate.
- Care for patients with complex needs.
- Support delivery of PCN service specifications.
- Collaborate with clinical pharmacists, first contact practitioners, social prescribing link workers, and care coordinators.
- Participate in MDT meetings and case discussions.
- Maintain high standards of clinical care and safety.
- Participate in clinical audit, quality improvement and significant event reviews.
- Comply with GMC standards, Practice and PCN policies, and information governance requirements.
Qualifications:
Essential:
- Fully qualified GP.
- Registered with the GMC and on the GP Performers List.
- Experience in UK General Practice.
- Excellent communication and interpersonal skills.
Desirable:
