About the Opportunity: RhynoCare is seeking an experienced Nurse Practitioner (NP) to work in Long Term Care facilities in Scarborough, ON. You will be hired and onboarded with RhynoCare as an Independent Contractor. At RhynoCare, we’re committed to your success. Position Overview: The Nurse Practitioner provides comprehensive, evidence-based primary health care to residents in Long Term Care, working collaboratively with physicians, Directors of Care, and interdisciplinary teams to support resident safety, quality of life, and continuity of care.
Responsibilities:
- Provide primary health care services to residents, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions within full NP scope of practice.
- Perform comprehensive health assessments encompassing physical, psychosocial, emotional, cultural, and functional dimensions.
- Order, interpret, and evaluate diagnostic investigations using evidence-based clinical reasoning.
- Develop, implement, and evaluate person-centered plans of care in collaboration with residents, families, and Substitute Decision Makers (SDMs).
- Select and implement appropriate pharmacological and non-pharmacological interventions to optimize resident outcomes.
- Collaborate and liaise with physicians, Directors of Care, and interdisciplinary teams in care delivery.
- Provide clinical leadership related to resident admissions and discharges, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and responsive behaviour management.
- Participate in resident care conferences and interdisciplinary rounds.
- Promote resident safety by identifying risks, mitigating harm, and addressing adverse events.
- Ensure accurate, timely documentation of assessments, diagnoses, care plans, interventions, and resident responses.
- Participate in quality improvement initiatives, Quality Committee meetings, and organizational efforts to improve resident care and safety.
- Support education and mentorship of staff, residents, and families to promote best practices.
- Dedicate approximately 80% of the time to direct resident care and 20% to education, research, and quality initiatives.
- Participate in after-hours and on-call coverage as required.
- Maintain continuing competence through active participation in the CNO Quality Assurance Program.
Requirements:
- Current registration with the College of Nurses of Ontario (CNO) in the Extended Class (Primary Health Care or Adult) and entitled to practice.
- Bachelor of Science in Nursing (BScN) required.
- Master of Science in Nursing (MSN) with Nurse Practitioner specialization preferred.
- One (1) year of clinical experience in geriatric, rehabilitation, or Long-Term Care nursing preferred.
- Availability to work on-site, including flexibility for evenings, weekends, and on-call coverage.
- Working knowledge of applicable Long-Term Care legislation, regulations, and standards of practice.
- Requires prolonged sitting, standing, walking, and reaching.
- Classified as “Medium” physical demand per National Occupational Classification (NOC).
- Ability to lift up to 50 lbs, with frequent lifting or carrying of up to 20 lbs.
- Ability to manage physical and emotional demands in a fast-paced clinical environment.
Compensation:
Pay: $85 / Hour
