Rhondda Cynon Taf Council is pleased to offer Rehabilitation Care Worker posts within the Rehabilitation Service. We are looking to appoint motivated and enthusiastic Care Workers to join the team in Rhondda Cynon Taff, based at Ty Elai, Rhondda, CF40 1NY.
The Rehabilitation service is a joint working initiative of health and social care comprising Occupational Therapists, Physiotherapists, an Occupational Therapy Assistant (OTA), Assistant Therapy Practitioners (ATP), a Team Lead and a highly trained team of social care staff. You will be supporting individuals through the delivery of an up to 6-week programme of support, to provide opportunities to recover from ill health and prevent episodes of health deterioration where possible.
Sometimes, following a spell in hospital, people can lose confidence and be worried about their safety and ability to cope at home. Rehabilitation Services offers individuals reassurance and encouragement to maintain, regain or learn basic living skills, providing support to enable people to reach their full potential whilst preventing or reducing the need to move onto longer-term community services. The Rehabilitation service works across the Rhondda Cynon Taff areas.
The Team has an emphasis on multi-agency working and you will be expected to work closely with colleagues from Health and a number of other agencies. Successful candidates will bring strong communication skills to ensure positive outcomes for the individuals with whom we work. The role involves providing practical support and encouragement to individuals undertaking a programme of support. The successful candidate will have regular 1:1 sessions, observations of work practice and be involved in Team Meetings.
