Mystic Healthcare, a 100 bed SNF located in Mystic, CT., is currently seeking a Physical Therapist to join our team of healthcare heroes.

Responsibilities

  • Review request for physical therapy and physicians\’ orders
  • Completes screening
  • Interviews residents to determine treatment type and schedule therapy
  • Participate in the development of resicent assessments (MDS) and care plans
  • Brief resident of procedures involved in pt and prepare necessary equipment
  • Assist in transporting residents to and from therapy
  • Determine proper equipment usage, applicantion and body position, and make adjustments as necessary in the administration of treatments
  • Ensure that all therapist notes are informative and descriptive of the care provided
  • Consult with physician/staff as necessary
