Mystic Healthcare, a 100 bed SNF located in Mystic, CT., is currently seeking a Physical Therapist to join our team of healthcare heroes.
Responsibilities
- Review request for physical therapy and physicians\’ orders
- Completes screening
- Interviews residents to determine treatment type and schedule therapy
- Participate in the development of resicent assessments (MDS) and care plans
- Brief resident of procedures involved in pt and prepare necessary equipment
- Assist in transporting residents to and from therapy
- Determine proper equipment usage, applicantion and body position, and make adjustments as necessary in the administration of treatments
- Ensure that all therapist notes are informative and descriptive of the care provided
- Consult with physician/staff as necessary
