Role overview: You will deliver face-to-face Continuing Healthcare (CHC) assessments, coordinate MDT input, complete eligibility documentation, and support review/dispute processes in line with the National CHC Framework.
Key responsibilities:
- Complete CHC assessments and reviews (patient facing)
- Produce clear, evidence-based reports and recommendations
- Liaise with clinicians, social care, families and providers
- Maintain accurate documentation and case records
Essential:
- Active NMC registration (RGN/RMN/RNLD)
- Proven CHC assessment / review experience
- Excellent written communication and decision documentation
- Ability to work on-site in Kettering
