Role Overview The successful candidate will undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs. This will include face-to-face assessments across the Birmingham and Solihull areas, with report writing completed either from home or the office. Key Responsibilities * Complete CHC and FNC assessments for adults with complex health needs * Undertake face-to-face assessments within Birmingham and Solihull * Complete DSTs, Checklists, Fast Track assessments and reviews * Work with patients, families, carers, advocates and multidisciplinary teams * Prepare clear, accurate and comprehensive assessment reports * Support CHC review panels and funding decision-making processes * Manage a complex clinical caseload independently * Liaise with care homes, local authorities, acute trusts and community services * Ensure assessments are completed in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care Essential Requirements * Registered Nurse with current NMC registration * Strong experience in CHC / Continuing Healthcare assessments * Experience completing DST, Checklist, Fast Track and FNC assessments * Good understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care * Experience working with adults with complex needs, long-term conditions, disabilities, palliative care or end-of-life care * Excellent report writing and communication skills * Ability to work independently and manage a complex caseload * Ability to travel across Birmingham and Solihull for face-to-face assessments Desirable Requirements * Previous experience working within an ICB, CCG or NHS CHC team * Experience presenting cases to CHC panels * Knowledge of safeguarding, DoLS, Care Act and Mental Capacity Act * Strong stakeholder management and negotiation skills
