We're looking for a compassionate and proactive Support Worker to join a dedicated team supporting veterans and individuals who have experienced or are at risk of homelessness. This is a rewarding opportunity to make a genuine difference by helping people with complex needs move towards independent living.
What you'll be doing:
- Managing a small caseload of 5–8 customers, allowing you to provide high-quality, person-centred support.
- Developing, reviewing and updating support plans and risk assessments.
- Supporting people with complex needs, including mental health, substance misuse and housing-related issues.
- Working closely with external agencies, landlords, healthcare professionals and support services.
- Promoting independence, wellbeing and positive outcomes for every individual you support.
- Maintaining accurate records and contributing to safeguarding and risk management processes.
