The predicted salary is between 24000 - 36000 Β£ per year.

We’re looking for a compassionate person to join our trusted team and support us on an ad-hoc basis when the team needs an extra helping hand. You’ll provide support as part of our Wellbeing Service to customers, working alongside Adult Social Care to vulnerable people. You’ll connect them to local resources and services that help them to maintain their independence and live comfortably at home, for as long as they’d like.

Job Requirements

  • Conduct holistic assessments with customers, using an outcome star to create and implement action plans that ensure their happiness, safety, and wellbeing at home.
  • Carry out both ad-hoc and regular visits to individuals in the community identified as needing extra support, completing tasks as requested. These tasks may include checking that homes are warm and safe, assisting customers with opening and managing mail, supporting them with appointments, and ensuring they have access to medication and food.
  • Connect customers with local community groups and resources to help meet their needs and enhance their wellbeing.

What We’re Looking For

  • Experience of support work, ideally within a health or social care setting.
  • Excellent communication skills, showing compassion and empathy.
  • Understanding and respecting individual needs while providing personalised support in a positive and supportive manner.
  • An aptitude for working with vulnerable and/or older people.
  • Ability to work under pressure and manage your daily tasks.
  • Excellent ICT skills.
  • Excellent administrative skills.
  • Excellent knowledge of local services including national and local charities.
  • Someone who is flexible throughout the week and able to work weekends.
  • A full UK driving licence suitable for business use.
  • DBS required.
