Revelation Social Care Ltd is seeking a highly organised, compassionate, and skilled Care Plan Lead to join our Outstanding‑driven team. This is a key role within our service, responsible for completing high‑quality assessments, developing person‑centred care plans, and ensuring our documentation meets CQC standards of Safe, Effective, Caring, Responsive, and Well‑Led. You will work closely with the Registered Manager, families, and external professionals to ensure every client receives personalised, safe, and consistent support. This role is ideal for someone with strong assessment skills, excellent written communication, and a passion for delivering exceptional care.

Key Responsibilities

  • Assessment & Care Planning
    • Complete holistic assessments for new clients, including personal care, mobility, emotional wellbeing, risks, and environmental needs.
    • Develop clear, person‑centred care plans that reflect individual preferences, routines, risks, and outcomes.
    • Update care plans following reviews, incidents, or changes in need.
    • Ensure all documentation is CQC‑compliant, accurate, and reflective of best practice.
  • Risk Management
    • Complete and review risk assessments (falls, moving & handling, behaviours, medication, environment, lone working).
    • Work with the Registered Manager to implement safe systems of work.
    • Ensure carers understand and follow risk management strategies.
  • Quality & Compliance
    • Maintain high standards of documentation aligned with CQC expectations.
    • Support the Registered Manager with audits, reviews, and governance tasks.
    • Ensure care plans are updated promptly and reflect real‑time changes.
    • Participate in quality assurance checks and contribute to continuous improvement.
  • Client & Family Liaison
    • Build strong, trusting relationships with clients and families.
    • Attend care reviews, MDT meetings, and professional discussions.
    • Ensure families feel informed, supported, and involved in care planning.
  • Staff Support & Training
    • Brief carers on new care plans and changes.
    • Support staff understanding of person‑centred care and risk management.
    • Provide guidance on documentation and best practice.

  • Experience in care planning, assessments, or senior care roles.
  • Excellent written and verbal communication skills.
  • Strong understanding of CQC standards and person‑centred care.
  • Confident in completing risk assessments and care documentation.
  • Ability to work independently and manage your own workload.

  • Full UK driving licence preferred.
