Rethink Mental Illness is recruiting a Care Navigator to support individuals transitioning from prison to the community. You will deliver person-centred care navigation, connect clients with health services, and coordinate multi-agency release planning.
The role requires NVQ3 in Health and Social Care, strong communication, and the ability to manage a high-volume caseload in a prison setting. 35 hours per week, full-time, in HMP Millsike with TEWV and Humber NHS partners.
