Rethink Mental Illness in Durham is seeking an Apprentice Health and Wellbeing Coach to join the Prisons and Criminal Justice Care Navigation team. You will combine work-based learning with a Level 2 Diploma, supporting individuals before and after release from custody to reconnect with healthcare services and improve wellbeing.
Working with Care Navigators and Health and Wellbeing Coaches, you will help people access community services, develop care plans and promote wellbeing.
