We’re expanding our Supported Living service—and we’re looking for an enthusiastic Care Co-ordinator to join our Chippenham branch and help shape the future of our service.
The Role
As a Care Co-ordinator, you will be at the heart of our Supported Living service. You’ll work closely with Local Authorities, Social Care teams and Health professionals to ensure our customers receive consistent, high-quality care. This is a varied, fast-paced role where no two days are the same. You’ll be responsible for coordinating staff, building strong relationships, supporting the recruitment process, and ensuring compliance with company policies and care standards.
Key Responsibilities
- Support the continued growth and development of our Supported Living service
- Allocate work effectively, ensuring rotas are well planned, travel-efficient, and fully covered before leaving the office
- Collaborate with the recruitment team and ensure safe, values-based hiring practices
- Arrange mentoring and shadow shifts for new starters
- Build strong relationships with staff and understand their skills, strengths, and availability
- Maintain up-to-date training records and ensure staff compliance
- Promote an “always striving for outstanding” mindset
- Manage holidays, sickness, and absence, supporting colleagues to meet expectations
- Set up new customers on our systems with accurate, detailed information
- Participate in on-call duties
- Handle confidential information with professionalism and integrity
- Uphold Retain Healthcare’s ethos, values, and commitment to high-quality care
About You
We’re Looking For Someone Who Is:
- Experienced in a similar care coordination role (NVQ Level 3 in Social Care preferred)
- A confident communicator with strong leadership skills
- Highly organised, methodical, and able to meet deadlines
- Motivated, forward-thinking, and proactive
- Skilled at conflict resolution and problem-solving
- Competent with IT systems, Microsoft Office, and databases
- In possession of a full UK driving licence
