Case Management is described by The Case Management Society UK as a collaborative process which assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual's health, social care, educational and employment needs, using communication and available resources to promote quality cost-effective outcomes.

You will be providing a Case Management service to our clients that is devoted to the coordination, rehabilitation, care and support of people with complex clinical needs. This will be done within the ethos of the company.

When delivering Case Management, Resurgence Case Management keeps the client at the centre of all activities. We have an ethos that maintains a holistic approach and strives to thoroughly understand the needs and circumstances of each individual and their family; so that effective solutions can be pursued that will work for their individual and unique situation. This will include acting as an advocate for the client, as well as facilitating and coordinating effective input from fellow professionals working with them. While being effective and efficient in delivery of high-quality individualised Case Management, our ethos is based also around treating people with kindness, respect, dignity and compassion.

Training

You will be required to complete all mandatory training as per the current Resurgence Case Management Policies. This will include a range of online modules as well as some in-person training. You will also be required, as part of your induction, to shadow our other, more experienced case managers, to gain knowledge and experience. On some occasions, as part of your CPD, you will be invited and expected to travel to training events held by external organisations.

General Responsibilities

  • To build a positive, productive, supportive relationship with clients and their families.
  • To have a good understanding of both the case management industry and legal system in relation to personal injury law and the litigation process.
  • To have experience of working closely with clinical and other professionals to build lasting relationships that ensure a positive outcome for our clients.
  • During your employment, you will devote the whole of your time, attention and skill during normal working hours to the carrying out of your duties under this contract, and you will not engage in any activities which would conflict with the proper performance of your duties or with our business interests.
  • Comply with the rules, regulations and policies of the business.
  • Use your best endeavours to promote, develop and protect our business, interests and reputation.
  • To be responsible for managing a portfolio of clients, maintaining and building solid working relationships.
  • To ensure a high level of client satisfaction is achieved through effective relationship management.

Key Responsibilities as Assistant Case Manager

  • You will be expected to represent Resurgence Case Management in a positive and professional manner in your role as Assistant Case Manager.
  • To follow the Case Management Frameworks to ensure compliance and consistency.
  • To assist the lead Case Manager in co-ordinating the MDT, and any other tasks in relation to Case Management and support of clients.
  • To complete all mandatory training.
  • To identify any areas of your work where you require any further training and discussing this with your manager/mentor.
  • To ensure good communication between yourself and the Case Manager regarding any changes or updates on cases.
  • To complete all necessary paperwork/tasks according to Resurgence Case Management's policies.
  • To maintain regular communication with other members of the Case Management team regarding areas such as recruitment and management of support workers.
  • To ensure CQC compliance standards are met.
  • Assisting with supervision, management and training of support workers on a regular basis and being a key point of contact for them as required.
  • To ensure a high level of client satisfaction is achieved through effective relationship management.
  • To ensure that your responsibilities to maintaining safeguarding, child and adult protection is integral to all activities you undertake, and you follow safeguarding guidelines closely and share immediately any concerns to your manager and colleagues.
  • To be responsible for maintaining your own clinical records and ensuring you attain to your employer's expectations with regard to fee earning targets. This will be detailed further in your contract of employment and reviewed during supervision.
  • To ensure that all of your activities (including record keeping) meet with CQC requirements and that you work with colleagues and the Company to ensure these requirements are met across the whole company structure and all relevant activities.
