Case Management is described by The Case Management Society UK as a collaborative process which assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual's health, social care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes.

You will be providing a Case Management service to our clients that is devoted to the coordination, rehabilitation, care and support of people with complex clinical needs. This will be done within the ethos of the company.

When delivering Case Management, Resurgence Case Management keeps the client at the centre of all activities. We have an ethos that maintains a holistic approach and strives to thoroughly understand the needs and circumstances of each individual and their family; so that effective solutions can be pursued that will work for their individual and unique situation.

This will include acting as an advocate for the client, as well as facilitating and coordinating effective input from fellow professionals working with them. While being effective and efficient in delivery of high-quality individualised Case Management, our ethos is based also around treating people with respect and dignity.
