Join Yes Care Services and make a real difference in the community! We are seeking a highly organized and committed Care Coordinator for our St Helens branch. This is a key role in ensuring seamless care delivery and supporting both our team and Service Users.
Key Responsibilities- Scheduling & Coordination – Manage rotas to ensure Service Users receive consistent, quality care
- Client Assessments – Support assessments and risk evaluations to develop tailored care plans
- Team Support – Guide and mentor care staff to maintain excellent standards
- Stakeholder Communication – Work closely with Service Users, families, and healthcare professionals
- Problem Solving – Handle scheduling challenges, absences, and urgent changes effectively
- On-Call Duties – Provide out-of-hours support (1 in 4 weekends and 1 weekday night)
