Join Yes Care Services and make a real difference in the community! We are seeking a highly organized and committed Care Coordinator for our St Helens branch. This is a key role in ensuring seamless care delivery and supporting both our team and Service Users.

Key Responsibilities
  • Scheduling & Coordination – Manage rotas to ensure Service Users receive consistent, quality care
  • Client Assessments – Support assessments and risk evaluations to develop tailored care plans
  • Team Support – Guide and mentor care staff to maintain excellent standards
  • Stakeholder Communication – Work closely with Service Users, families, and healthcare professionals
  • Problem Solving – Handle scheduling challenges, absences, and urgent changes effectively
  • On-Call Duties – Provide out-of-hours support (1 in 4 weekends and 1 weekday night)
