Serving the people of Bradford, Harrogate, Selby and Craven Districts. Carers’ Resource is excited to announce a new opportunity to make a real difference in the lives of carers. We are currently seeking a Carer Locality Worker supporting Airedale Collaborative Care Team (ACCT).
Hours: 23 hours per week
Role Overview: You will play a pivotal role in supporting unpaid carers through listening, advising, and advocating for them. You’ll conduct wellbeing reviews, needs assessments, identify and carry out required actions, and collaborate with local services. Additionally, you’ll work closely with Airedale Collaborative Care Team, engaging in outreach work to promote our services and generate referrals of carers from the team to our services. The aim of ACCT is to help individuals remain at home and avoid unnecessary admission to hospital as well as assisting with early discharge from hospital. It is likely that many of the individuals ACCT assist will have an unpaid carer supporting them. You may facilitate peer support groups in the community. Your work aligns with local safeguarding procedures, promoting wellbeing and positive change for carers.
Requirements: Own transport, clean driving license, and business class insurance is a requirement of the role.
