Role Overview

As a Carer Navigator you will identify and address the needs of unpaid carers whilst the person they care for is in hospital. You’ll play a pivotal role in supporting carers through listening, advising, and advocating for them in the hospital setting, liaising closely with health and social care staff in the hospital. You will be working alongside hospital and social care staff to improve patient flow and support a timely and positive experience of hospital discharge. Additionally, you’ll engage in outreach work to promote our services.

Key Responsibilities

  • Collaborate with health and social care staff to improve patient flow and facilitate timely discharge.
  • Engage in outreach work to promote Carers’ Resource services.
  • Collaborate with local partners and a dedicated team to build support for carers.

Work Location

Based either at Airedale General Hospital (some work from Skipton office) or Bradford Royal Infirmary (some work from Shipley office). Monday‑Friday required, with some weekend working possible. Includes a 37‑hour week with the salary mentioned above.
