As a Support Worker, you will support individuals in their own homes and local communities, focusing on independence, choice, dignity, and outcomes that matter to them. This may include supporting individuals to:
- Build confidence with everyday tasks
- Maintain routines at home
- Access their community and social opportunities
- Work towards personal goals and outcomes
Support is delivered in a way that promotes autonomy, wellbeing, and inclusion.
Key responsibilities:
- Supporting individuals to work towards agreed goals and outcomes
- Promoting independence, choice, and dignity at all times
- Supporting with daily living activities, including personal care and medication support where required
- Encouraging and recording progress using digital systems
- Building positive, professional relationships with individuals, families, and colleagues
- Following safeguarding, risk assessment, and safe working practices
- Communicating clearly and raising concerns appropriately
