Rennie Grove Peace Hospice Care is seeking a Senior Hospice at Home Community Nurse/Paramedic for 34.5 hours per week (3 shifts of 11.5 hours). You will respond to SOS calls visiting patients at home to provide palliative and end-of-life care, and lead assessment, planning and evaluation of individualized, patient-centered care.
As a lone worker you will coordinate shifts, visit schedules and ensure safety, while upholding the charity's mission and values and acting as an ambassador.
