Location: St Albans, Harpenden and Dacorum Locality Area (Travel between all Rennie Grove Peace offices will be required as part of this role.)

What you will do
The purpose of the role is to provide high quality specialist palliative practitioner care, skills, advice and support to patients, carers and those important to the patient. This role will support the delivery of planned and unplanned care across Rennie Grove Peace Hospice. Actively participate in the development of the organisation’s planned and unplanned community model of care. Offer advice, support and education to the primary health care team as part of an interdisciplinary team. The management of a specialist palliative planned care caseload within a set Primary Care Network (PCN) and cross‑cover other surgeries as required to maintain our service. Work from an interdisciplinary approach to care. Engage in the full range of activities consistent with the role of a Clinical Nurse Specialist/ Clinical Paramedic Specialist.

- Manual handling and/or heavy lifting.
- Full, current valid UK driving licence.
- Access to a vehicle which can be used for work purposes.

- Professional registration (e.g. NMC, GMC, HCPC, BACP).
