Role Overview

The role involves providing high-quality specialist palliative nursing care, advice, and support to patients, carers, and their families. The nurse/paramedic will support planned and unplanned care delivery across Rennie Grove Peace Hospice, working within a multidisciplinary team to ensure excellent standards of care and support physical and emotional well-being.

Main Duties and Responsibilities

  • Participate in developing the community Hospice at Home model of care.
  • Work as part of a multidisciplinary team to deliver personalized patient and family care.
  • Manage allocated patient cases daily, ensuring high standards of clinical practice.
  • Provide specialist palliative nursing care, including virtual and in-home reviews for higher acuity patients.
  • Collaborate with the wider hospice team and system partners to support patients' preferred place of care.
  • Enhance patient outcomes and experiences through coordinated care and emotional support.
  • Engage in audits and case reviews to improve service quality.
  • Represent Rennie Grove Peace Hospice Care in the community.
  • Support safe and effective service delivery across the geographical area as needed.
