We are seeking a qualified social worker to work alongside a team of multi-disciplinary staff in the provision of hospital discharge. Working alongside Social Workers, Occupational Therapists, and medical staff, you will undertake assessment, review, care coordination and practical interventions within the Renfrewshire area for patients being discharged from hospital. This role is based within the Older Adult Community Mental Health team, and you will be working with older adult psychiatric patients.
Primarily based at the Royal Alexandra Hospital, Paisley, the role may require travel to alternative sites. You will operate under the Council’s WORKSMART arrangements which promote mobile and flexible work practices from any council premises travelling throughout the Council area.
You must be able to use IT packages; possess effective assessment and writing skills as well as interpersonal skills; communicate effectively verbally and in writing, work as part of a team as well as on your own initiative, prioritise your own workload, motivated, committed to Social Work values and be able to travel throughout the Renfrewshire Area.
You must hold a BA (Hons) in Social Work or equivalent and be registered with the Scottish Social Services Council (or confirm intent to register). Applications from Mental Health Officers are desirable. An additional allowance is payable to practicing MHO’s.
Full training and support will be given to ensure that you are able to maintain the department’s commitment to continuous improvement. If successful, you will be required to undertake a Disclosure Scotland Check, the level of which will be determined by the duties of the post.
