Renfrewshire Health & Social Care Partnership is seeking a motivated and experienced Service Coordinator to play a key role in delivering high-quality Care at Home services and making a meaningful difference within the local community. This is an exciting opportunity to join a fast-paced and supportive environment where you will ensure people receive the right care, at the right time, in a way that promotes independence and wellbeing.
You will take on a central and highly influential role in managing referrals, coordinating services, supporting staff, and contributing to the ongoing development of Care at Home provision across Renfrewshire. A key focus of the role is the effective management of referrals and service demand across the Care at Home service, ensuring timely access to care and supporting safe hospital discharge.
You will oversee hospital discharge referrals, community referrals and waiting lists, ensuring demand is prioritised appropriately and care is allocated in a safe and timely way. Through this, you will play a vital role in managing the flow of care across the system and supporting continuity of care within the community.
You will be responsible for the daily oversight and prioritisation of referrals into the service, ensuring that resources are deployed effectively and that service provision remains responsive to changing demand. Using electronic scheduling systems, you will support staff to maintain accurate records and make informed decisions regarding the deployment of frontline workers, ensuring services are delivered efficiently and in line with assessed needs.
Alongside this, you will have effective leadership skills and ability to organise, develop and motivate people with proven experience of supervising, supporting and managing the workload of staff. You will provide leadership, supervision and support; it is desirable that you have experience of recruitment, induction, training and staff development processes, while promoting high standards of performance and practice.
We are looking for someone with the proven ability to build and sustain positive and appropriate professional relationships with colleagues, service users and family members. You should be able to work well under pressure, have experience of working effectively as part of a team within a demanding environment and be able to work on your own initiative.
You should have working knowledge of MS Office suite and other relevant IT systems, have experience of updating, managing, and maintaining data through the use of electronic systems and be keen to learn new technology. As well as proven experience in writing comprehensive reports including recommendations and improvements, written and verbal communication skills and the ability to tailor communications depending on the audience.
It is essential that you are in possession of an SVQ Level 3 in Health and Social Care or equivalent qualification or an agreement to undertake in post within a specified period, in order to achieve and maintain SSSC registration.
