Renfrewshire Council is seeking an Adult Services Co-ordinator for hospital discharge services. This role involves assessment, care coordination, and practical interventions for older adult patients being discharged from hospital.
Working within the Older Adult Community Mental Health team, you will collaborate with Social Workers and medical personnel and promote mobile work practices throughout Renfrewshire.
An SVQ Level 3 in Social Care or HNC in Care or equivalent is desirable, alongside strong communication and interpersonal skills.
