- Adult Services Coordinator
Working alongside Health and Social Care Professionals and the Care at Home service, you will undertake assessment, review, rehabilitation, goal setting and care management including SDS, across the Renfrewshire area.
You will operate under the Council's Worksmart arrangements which promote mobile and flexible work practices from any council premises travelling throughout the Council area.
You must be able to use IT packages; possess sound assessment and interpersonal skills, communicate effectively verbally and in writing; work as part of a team as well as autonomously; prioritise your own workload; be motivated and committed to Social Work values.
You must be able to travel independently throughout the Renfrewshire wide area.
An SVQ level 3in Social Care or HNC in Care or Occupational Therapy Support or equivalent is desirable as is the use of SWIFT.
Full training and support will be given to ensure that you are able to maintain the department's commitment to continuous improvement.
If successful, you will be required to undertake a Disclosure Scotland Check, the level of which will be determined by the duties of the post.
