The predicted salary is between 30000 - 40000 £ per year.
Working alongside Social Workers, Occupational Therapists, and medical staff, you will undertake assessment, review, care coordination and practical interventions within the Renfrewshire area for patients being discharged from hospital. This role is based within the Older Adult Community Mental Health team and you will be working with older adult psychiatric patients.
You will operate under the Council's WORKSMART arrangements which promote mobile and flexible work practices from any council premises travelling throughout the Council area.
You must be able to use IT packages; possess sound assessment and interpersonal skills; communicate effectively verbally and in writing, work as part of a team as well as on your own initiative, prioritise your own workload, motivated, committed to Social Work values and be able to travel throughout the Renfrewshire Area.
An SVQ Level 3 in Social Care or HNC in Care or Occupational Therapy Support or equivalent is desirable as is the use of SWIFT. Full training and support will be given to ensure that you are able to maintain the department's commitment to continuous improvement.
