You will take on a central and highly influential role in managing referrals, coordinating services, supporting staff, and contributing to the ongoing development of Care at Home provision across Renfrewshire. A key focus of the role is the effective management of referrals and service demand across the Care at Home service, ensuring timely access to care and supporting safe hospital discharge. You will oversee hospital discharge referrals, community referrals and waiting lists, ensuring demand is prioritised appropriately and care is allocated in a safe and timely way. Through this, you will play a vital role in managing the flow of care across the system and supporting continuity of care within the community.
You will be responsible for the daily oversight and prioritisation of referrals into the service, ensuring that resources are deployed effectively and that service provision remains responsive to changing demand. In return, you will join a committed and supportive team, with opportunities to further develop your leadership and operational skills. This is a highly impactful role within Care at Home services, offering the opportunity to shape how care is accessed and delivered across Renfrewshire, and to make a tangible difference to people's lives every day.
