The predicted salary is between 36 - 42 £ per hour.

General Duties: Participates in the delivery of Healthy Babies Healthy Children program services (HBHC). This includes but is not limited to home visiting, needs assessment, follow up, and referral to ensure appropriate and optimum levels of services for young families and to ensure optimum level of development for young children.

Qualifications, Skills and Experience:

  • Four (4) year undergraduate degree in Social Work from a recognized university;
  • Minimum of one (1) year experience in social work;
  • Membership with the College of Social Workers and Social Service Workers of Ontario;
  • Proof that you have been fully vaccinated against COVID-19;
  • Acceptable vulnerable sector criminal records check required;
  • Valid Ontario ‘G’ Driver’s License and own vehicle required.

Collaborating with Public Health Nurses (PHN) and Home Visitors (HV) to support levels of care and service; Providing social support and related services to clients through the provision of counselling (related to finances, housing, access to services, etc.), needs assessment and evaluation, and making recommendations for appropriate actions to ensure clients have information and skills to provide optimum health for themselves and their children; and Referring and/or advocating for clients to appropriate community agencies to respond to social and health needs.

Location: Pembroke office

Hourly Rate: $49.57 to $56.94 per hour (2025 rates), plus pro-rata vacation and 14% payment in lieu of benefits
