Our client, Bury Council, is looking for a Qualified Social Worker to join their Preparing for Adulthood team. As a Social Worker, you will support young people with care needs, disabilities, and complex transitions. You will hold a caseload of transition work and provide high-quality practice that promotes independence, aspiration, and inclusion.
Responsibilities:
- Drive excellent transitions planning aligned with the Preparing for Adulthood national framework
- Build effective partnerships across services, schools, colleges, and health
- Champion young people’s voices and ensure their aspirations underpin every decision
- Support the Team Manager with quality assurance, performance oversight, and developing best practice
- Model strengths-based, person-centred, and co-productive approaches
- Champion approaches that maximise independence, enabling young people to build the skills, confidence, and autonomy they need for adulthood.
