Our client Wakefield Council is looking for a Senior Social Worker to join their Adult Safeguarding Team.

Main Tasks

To undertake comprehensive needs assessments.

To plan, implement and review care plans within the framework of the Care Programme Approach.

To provide social work interventions to individual service users and carers as identified within the care plan.

To contribute to the effective assessment and management of risk and protection of others.

To liaise with the Probation Service and make referrals to the relevant MAPPA when appropriate.

To undertake care co-ordination and social supervision of service users living in the Wakefield locality.

To contribute to the provision of specialist multi-disciplinary treatment programmes.

To work with local and regional mental health services to meet service users needs and facilitate effective transfer of care and discharge.

To negotiate for and mobilise resources both locally and regionally.

To liaise with other relevant Health and Local Authorities, agencies and professionals as appropriate.

To provide reports for Mental Health Review Tribunals; Managers Hearings and the Home Office.

To present evidence at Mental Health Review Tribunals, Managers Hearings and other formal meetings as required.

To undertake a lead role in implementing the YCFP's Child Visiting Policy and ensure any child welfare issues are identified and addressed in accordance with Safeguarding Children Policies and Procedures.

To assist service users to obtain their full welfare benefit entitlement.

To undertake investigations under the Multi-agency Adult Protection Policy.

The postholder will fully participate in supervision and appraisal as required.

To supervise Social Work and Approved Social Worker student placements.

To supervise the work of Mental Health Support Workers when appropriate as part of an agreed care plan.
