You will manage a caseload of adults, delivering strengths-based assessments, support planning, safeguarding interventions, and reviews to promote independence, wellbeing, and positive outcomes.
As part of the role you will collaborate with statutory, voluntary and community partners, attend peer reviews, maintain records, and provide guidance to colleagues.
