Our client Rochdale Borough Council is looking for an Adults Social Worker to join their Hospital Discharge team. Hospital Social Work requires you to be a confident, dynamic fast paced individual with excellent communication skills. You will have experience and motivation to work with adults, their carers and develop positive relationships with health colleagues across the hospital sites. Working within timescales and deadlines is essential for this role.

What are we looking for?

  • You will be an experienced social worker and will have current hospital social work experience.
  • You will have excellent knowledge of the Mental Capacity Act and confident experience in completion of Mental Capacity Assessments/Best Interest Processes.
  • You will promote a strength based approach to assessments and are able to think creatively with solutions to support planning to meet care needs on discharge from hospital as within the objectives of the Care Act 2014.
  • Additionally, you are expected to promote the independence and well-being of service users and carers, utilising natural support and the voluntary sector.
  • You will be an effective multi-tasker with excellent organisational skills and the ability to work flexibly to meet the needs of the service.
  • You will be motivated and forward thinking, who can demonstrate professionalism and enthusiasm.
  • You will be a team player.
  • You will have excellent interpersonal and communication skills.
  • You will have excellent ICT skills and be able to use a range of packages including Word, Excel and Outlook.
  • You will have a real attention for detail and be able to maintain accurate and detailed records using both manual and electronic systems.
  • You will be expected to visit all hospital sites local to Rochdale and out of area as with the needs of the service and access to a car is essential.
  • You will be expected to be office based and respond to the demands on the day, but there is an opportunity for some home working.
  • You will be expected to be part of the duty team.
