We are looking for a dedicated and motivated Social Worker to join the Longer Term Care Team, supporting adults to achieve greater independence and improved quality of life. The Role

As a Social Worker within the Longer Term Care Team, you will manage your own caseload while working collaboratively with citizens, carers, healthcare professionals, and partner organisations to deliver high-quality, outcome-focused support.

Key responsibilities include:

  • Undertaking assessments in line with statutory duties and relevant legislation.
  • Developing, implementing, reviewing, and monitoring person-centred care and support plans.
  • Supporting adults with physical disabilities, mental health needs, illness, or age-related frailty to achieve their personal outcomes.
  • Working closely with carers, families, statutory services, and voluntary organisations to promote independence.
  • Managing a varied caseload, prioritising work effectively.
