The Role
As a Social Worker within the Longer Term Care Team, you will manage your own caseload while working collaboratively with citizens, carers, healthcare professionals, and partner organisations to deliver high-quality, outcome-focused support.
Key responsibilities include:
- Undertaking assessments in line with statutory duties and relevant legislation.
- Developing, implementing, reviewing, and monitoring person-centred care and support plans.
- Supporting adults with physical disabilities, mental health needs, illness, or age-related frailty to achieve their personal outcomes.
- Working closely with carers, families, statutory services, and voluntary organisations to promote independence.
- Managing a varied caseload, prioritising work effectively.
