Our client, Manchester City Council is looking for a Qualified Social Worker to join their Adults social Services.
A social worker is required to support the HDTA Central team for a 12-month period covering Intermediate care at Willowbank. The role involves providing direct support to service users and their families in line with statutory duties and national/local policies, ensuring positive outcomes for citizens. Working closely with MLCO partners and other stakeholders, the role holder will assess and manage current and potential risks, and design person-centred support plans that promote independence.
Key responsibilities include:
- Actively contributing to MDT discussions, particularly around Estimated Discharge Dates (EDD), length of stays etc.
- Supporting patients and families to manage expectations during the discharge and care-planning process.
- Collaborating with therapy staff to gather assessment information, including for high-value care packages.
- Keeping the flow lead informed of patient progress and escalating social care concerns through senior practitioners and the team manager as needed.
- Remain on HDTA weekend rota (aligned with unit coverage).
