Adult Social Worker - Hospital Team Job Description Job title: Adult Social Worker - Hospital Team Role purpose: To provide professional social work support to adults in hospital, helping patients and their families address social care, safeguarding, wellbeing and discharge needs. The role involves working closely with health and social care professionals to ensure patients can leave hospital safely and with appropriate support. Key responsibilities Complete assessments of adults' social care and support needs. Develop and review care and support plans. Support safe and timely hospital discharge. Assess patients' ability to manage safely at home and identify appropriate support. Arrange or coordinate care packages, residential or nursing placements and community support where appropriate.
