Hackney Council is hiring a Reviewing Officer to join the Access & Duty team.

The role involves carrying out strengths-based reviews and reassessments for people with long-term conditions, visiting service users at home or in care homes, and maximizing independence by applying an outcome-based support plan.

The post includes budgeting, monitoring residential placements, and providing information and signposting to universal services. DBS disclosure will be required.
